While traveling westbound on US 36 into Lyons, turn right on 2nd Avenue. The clinic is in “The Little Yellow House” half a block up 2nd Ave on the left side of the street. There is ample parking in front and the building is handicapped accessible.

The office is located in the Steelyards off 30th St. The entrance to 2500 is just south of the entrance to Fresh Thymes Eatery. Take the stairs or the elevator to the second floor and look for Suite 200.
